This project explored the barriers and enablers to interprofessional collaborative family practice teams (CFPTs) in Nova Scotia to improve access to primary care. A literature review and provincial survey helped identify and prioritize key factors influencing collaboration. Using the Consolidated Framework for Implementation Research, findings were organized into five domains, including system-level and team-level considerations. A knowledge-sharing event then brought together CFPT members, policymakers, patients, and caregivers to identify strategies to enhance enablers and address barriers in team-based care.
