The Hospital to Home Pharmacy Project is a research study looking at how patient experiences can be improved when pharmacists are directly involved in helping them transition home after a hospital stay. Patient partners engaged in the research helped Dr. Goodine realize that discharge was not the right time to overwhelm patients by teaching them about their medications.
The research project has now expanded to 17 pharmacies in the greater Fredericton, New Brunswick, area. The research team has produced a patient-friendly pamphlet and are currently testing a computer-generated medication calendar that pharmacists can give patients to take home and which show them when to take specific medications and how much of them to take. The research team is currently collecting data to evaluate the impact of the pilot project and, eventually, looking to expand it from coast to coast. (CIHR)
