Chronic diseases are affecting more and more Nova Scotians, and many people are living with multiple conditions at once. Through a province-wide survey, this project will gather patient experiences to understand what makes it easier or harder to manage chronic illness and access care. By focusing on questions that matter to patients, the findings will help improve supports, guide system changes, and strengthen chronic disease care across Nova Scotia.
Eligibility
- Lived or living experience managing one or more chronic health conditions in Nova Scotia (either as a patient or caregiver).
- Experience accessing or navigating the Nova Scotia healthcare system.
- Interest in improving chronic disease care and patient experiences.
- Ability to participate in virtual meetings and review materials.
We welcome individuals from diverse backgrounds, including those who may face structural barriers to care (e.g., rural residents, equity-deserving communities, people managing multiple chronic conditions). Diversity of experience is valued.
Time Commitment
Engagement will run from April 2026 to April 2027. The team will meet virtually for 1 hour per month. Materials will be shared for review with approximately 30–60 minutes of preparation time to review materials in advance.
