Transitions in Care: Understanding Healthcare Use Before and After Moving from Pediatric to Adult Care for Chronic Conditions

This study looks at how young people with health conditions like Inflammatory Bowel Disease, Juvenile Idiopathic Arthritis, Cystic Fibrosis, and Diabetes use healthcare services before and after they transition from pediatric (child-focused) care to adult care. The goal is to better understand their healthcare needs during this important time and identify ways to improve their care experience.

We are looking for Patient/Public Partners to share their insights and lived experiences. As a partner, you would:

  • Attend meetings.
  • Help explain or understand the results
  • Collaborate on ways to share results with communities, healthcare providers, and policymakers.
  • Suggest practical recommendations to improve the transition from pediatric to adult care.

Eligibility

  • A personal experience transitioning from pediatric to adult healthcare services for a health condition such as Inflammatory Bowel Disease, Juvenile Idiopathic Arthritis, Cystic Fibrosis, or Diabetes, or
  • a Caregiver or family members who have supported someone through this transition.
  • Lives in Nova Scotia

Time commitment
This project will span two years, with all meetings held virtually. We will meet at least once per quarter (every 3 months) for one hour, and materials will be shared in advance for review. We will collaborate with the Patient Public Partner to determine how they would like to be involved in the process.

Appreciation
Compensation will be offered.
