Implementing a Nurse-Practitioner Led Screening Tool for Social Determinants of Health in a High-Needs Area: Implications for Practice and Policy

Researchers are looking for a patient advisor for a project focused on a new screening tool to better understand and address patients’ social needs.

The Nurse Practitioner leading this study is currently using the screening tool, called the Health Leads questionnaire, in her practice. This tool helps identify patients’ social needs, such as housing or food insecurity. Now, they want to learn:

  1. How well the tool helps the Nurse Practitioner understand patients’ needs.
  2. How they can improve the process of using it in care.

As a patient advisor, your role would be to:

  • Help create materials to inform the public about this tool and how it can improve care.
  • Review the information collected so far.
  • Share your insights to make the process better and more user-friendly for patients.

Eligibility

  • Live in Nova Scotia, specifically someone who is familiar with the Dartmouth area
  • Comfortable sharing their perspectives on how social needs (housing, employment etc.) can affect health

Time commitment
The patient advisor would attend (virtually) up to two research meetings (1 hour each) and may be asked to provide input on materials to be shared with the public (up to 5 hours).

Compensation will be offered.
