The Canadian Home Care Association (CHCA) invites you to participate in a national consultation to help refine and validate an Integrated Care Decision-Support Tool designed to support more coordinated, team-based care for older adults living with complex health and social needs. This project is supported by the CIHR Institute of Aging Voluntary Sector Knowledge Mobilization Support Grant.
Purpose of the Project: Older adults frequently live with multiple chronic conditions and complex care needs that require coordination across home care, primary care, and community services. While integrated care is widely recognized as essential, organizations often lack practical tools to support collaboration across sectors. This project will refine and validate CHCA’s Integrated Care Decision-Support Tool to help organizations strengthen integrated care planning and implementation across care settings. Your input will help ensure that the tool is practical, relevant, and responsive to real-world needs.
Eligibility
We are looking for Persons with Lived Experience which may include experience as a provider, planner, older adult, or caregiver in the Maritimes.
Time Commitment
Participation will run from July 2026 through March 2027 and will require approximately 10 hours in total. This includes 2–3 hours of virtual engagement: an optional one hour project open house in July, a mandatory one hour consultation session in October, and an optional feedback consolidation/wrap up session in January.
You can also expect to spend 4–8 hours between July and October reviewing and testing the decision support tool at your convenience, and sharing your feedback through an online survey ahead of the October virtual consultation.
Participation and Time Commitment Include:
Attending an optional virtual project open house on July 23 at 12pm ET to introduce the decision‑support tool (recording will be shared afterward)
Reviewing/testing the tool and its supporting materials, and assessing its relevance and application within your specific context
Completing an online survey to provide feedback
Participating in a virtual consultation on October 6 at 12pm ET
Attending an optional virtual feedback consolidation/wrap‑up session in January (date/time TBD) to present changes made based on participant feedback (recording will be shared afterward).
Compensation
Participants will be eligible to receive a $100 amazon gift card in recognition of their time and contributions to the consultation activities. The honorarium will be provided to participants who complete all mandatory project activities, including reviewing/testing the materials, completing the survey, and participating in the consultation session.
