About the project

Chronic conditions affect a growing number of Nova Scotians, and many people are living with multiple conditions at once. Managing these conditions can be challenging, people may experience difficulty accessing the right care at the right time, or gaps in the treatments and services they need.

This project aims to better understand those challenges through a province-wide survey administered through Nova Scotia’s Engage4Health platform. We will gather patient experiences to understand what makes it easier or harder to manage chronic illness and get the care that’s needed. People with lived and living experience of chronic disease will be part of our research team, and will play a key role in shaping the survey by helping to decide which issues matter most. These findings will provide meaningful insights to inform health system changes and strengthen chronic disease care across Nova Scotia.
