Researchers at Dalhousie University are looking for two patient partners to join their research team for a cross-Canada study related to the transition from pediatric to adult care. Supporting youth who are transitioning from pediatric to adult care is a health system priority. Many transition in care (TiC) innovations (i.e., guidelines, pathways) exist. Currently, we do not know how to ensure these innovations are used in the long-term. Knowledge translation interventions (such as education and clinical champions) are used to promote the implementation of these innovations into clinical practice.
This study is examining what, when, and how knowledge translation interventions work or do not work, and under what contexts, for the implementation and sustainability of TiC innovations. A panel of people with lived and professional experience will work together as equal partners to co-produce each phase of this research project.
Eligibility
- Current or former youth patients of IWK Health, aged 15-24 years, who have been diagnosed with a chronic health condition.
- Caregivers (including parents or guardians) of youth who will be transferred or have already been transferred to adult healthcare.
- Youth and caregiver partners will be responsible for providing their lived experience with the transition to adult healthcare.
Time Commitment
Youth and caregiver partners will be invited to meet with the panel members twice annually and as needed. Partners will have the opportunity to contribute to various project tasks, approximately one-hour time commitment per month. The level of engagement will vary for each research phase, activity, and partner’s capacity and interest. Multiple methods will be offered to provide feedback and input.
Compensation
Youth and caregiver partners will be compensated for their time. With permission, all partners will be recognized by name in the acknowledgement section of project publications. Authorship on project publications will be offered to those who make substantial contributions.
