Implementing and evaluating a framework to support the transition from pediatric to adult care

Researchers at IWK Health are looking for 2-3 patient partners to join their research team. This study is exploring the use of a Transition in Care Framework at IWK Health and Nova Scotia Health, from the perspective of youth, caregivers, and health care providers. Patient partners will provide project guidance on data collection procedures, interpretation of study findings, and sharing the results of the work.

Eligibility

Former pediatric patients who have been diagnosed with a chronic health condition and have transferred from IWK Health to adult care are invited to be patient partners. Patient partners can be from anywhere in Nova Scotia, New Brunswick, or Prince Edward Island.

Time Commitment

Patient partners are invited to attend monthly 30-minute to 1-hour meetings with the research team. Partners may also be asked to provide input over email. We are looking for patients partners to join our team in February 2026 and the work will be completed by April 2027.

Compensation

Patient partners will be compensated for their time.
